According to the final coroner’s report, released today, Cory Monteith was shooting heroin and drinking champagne on the night he died. The official findings say responders arrived to Cory’s Vancouver hotel room shortly after noon on July 13 and found there a “spoon with drug residue and a used hypodermic needle.” They also discovered two empty champagne glasses and bottles. The coroner lists Monteith’s cause of death as “mixed drug toxicity, involving intravenous heroin use combined with the ingestion of alcohol.” Because Monteith, 31 at the time of his passing, had been off the wagon since April, the coroner notes that his tolerance for foreign substances may have been lower than in the past. He likely believed his body could handle what he was putting in it. The report also concludes that the Glee star “had a history of illicit drug use with intermittent periods of rehabilitation and of abstinence from drugs.” Glee Season 5 will pay tribute to Monteith a special, drug-themed episode on Thursday, October 10.

Funeral Home Puts Teenager’s Ashes In Walmart Bag We understand sometimes you have to improvise, but he could have found something better than a Walmart bag for her son’s ashes. According to Mail Online A grieving mother in Ohio is disgusted after she found her son’s cremated remains wrapped in a Walmart bag. Nancy Bronner of Amelia, Ohio lost her 17-year-old son T.J. Mitchell last September due to a heart condition. Ever since he was 15, her son knew he wanted to be cremated when he died. So when he passed away, his remains were cremated and his ashes split among family members into four urns. But Ms Bronner was surprised when she opened her urn. ‘I just wanted to hold my baby one last time,’ she told WCPO. ‘So I opened the urn, and found Walmart bag in the urn. The funeral home had put my son’s ashes in a Walmart bag.’ She calls the act disrespectful, unprofessional and a desecration of her son’s remains. T.J. was cremated at McDaniel Funeral Home in Dry Ridge, Kentucky by Bob McDaniel, who is also the Grant County coroner. The teens remains were being split between four urns – something the funeral home doesn’t usually do – and no one in the family supplied bags for the urns which do not seal. There is no good explanation for this. In an attempt to make sure no ashes spilled out, Mr McDaniel used bags that he had on hand to wrap the ashed in the urns. He says he took scissors to the shopping bags to remove any store logos before wrapping the ashes. T.J.’s father Thomas Mitchell, and Nancy Bronner’s ex-husband, says using the Walmart bags could be in bad judgement but doesn’t believe the coroner acted maliciously. Mr Mitchell still keeps his sons remains wrapped in the grocery bag the funeral home provided. Ms Bronner has since taken her son’s ashes to another funeral home to be placed in a clear plastic bag. Ms Bronner, who says she is in therapy over her son’s death, has contacted the Kentucky Attorney General’s Office and the Board of Embalmers and Funeral Directors to complain about the funeral home. ‘I don’t care who you are, it’s not right to put a human person’s ashes in a Walmart bag,’ she said. ‘It is disrespectful. You don’t do that to a parent who has lost their child. You don’t do that to anyone. I just want to make sure that this never happens again to another person,’ she said. SMH. The death of Lisa Robin Kelly on August remains a mystery to authorities , with new questions being raised just a few days after this actress passed away at a rehab facility. TMZ has discovered that both the L.A. County Coroner and the L.A. County Sheriff officers are conducting their own investigations and that the main point of contention is this: The County Coroner never received a phone call from the facility where Kelly died . Moreover, the Coroner never learned of the passing until four hours after the facility pronounced Kelly dead (at 8:07 a.m.), with the physician on scene claiming Kelly died from an embolism. But how would he have known that? It’s impossible to look at a corpse and know one passed away from an embolism. It’s unclear whether there were any drugs in Kelly’s system, although the facility does does claim she was on detox drugs of some kind. No new information is expected to be released, per the Sheriff’s orders, until this investigation is complete. But in the meantime, Kelly’s ex-husband and boyfriend continue to point fingers at each other regarding who is more to blame for Kelly falling so far off the wagon. It’s a sad situation all around. Teen Commits Suicide After Facebook Post According to Mail Online A 19-year-old man committed suicide following a sarcastic Facebook post about his arrest warrant by his local sheriff’s office, his family said. The sister of Andrew Cain is demanding an apology from an Idaho sheriff’s deputy she says harassed her brother on the social networking site in the days before his death. Alise Smith said her brother’s decision to end his life was his own, but she believes the comments that he received about arrest warrants in Latah County became too much for him. The sheriff’s office in Idaho posted a picture of Andrew Cain with the message: ‘We have decided that Andrew Cain is no longer the Wanted Person of the Week… he is the Wanted Person of the Month of June. Congratulations!’ Smith, of Pullman, Washington, said a deputy also sent private messages to her brother on Facebook. Cain fatally shot himself Sunday, Whitman County Coroner Pete Martin said. Sheriff Wayne Rausch told the Associated Press that a deputy did exchange messages with Cain, but the printout he saw showed it was Cain who initiated the exchange by telling the deputy he liked his new wanted poster. The deputy replied that if Cain turned himself in, he’d give him a copy. ‘How this could be construed as taunting or rude or harassment or anything like that, I’m at a loss,’ Rausch said. Smith said she understood the ‘wanted poster’ but felt the ‘congratulations’ comment and the private messages were an abuse of power. Rausch said he told the employee who added the ‘congratulations’ to the post that it was not appropriate and not to do it again. Smith told KLEW-TV that she received a text from her brother early last week that said he felt like putting a bullet in his brain. The message included a screen shot of Facebook messages from a deputy, Smith said. ‘Eventually, it all just got too much to handle because other people were texting him and messaging him on Facebook and he just couldn’t handle all of the people telling him how awful a person he was,’ Smith told KLEW. Both Martin and Rausch called the death a tragedy. ‘That he felt necessary to end his life over facing consequences is tragic,’ Rausch said in his statement. We’re not being mean, but if that Facebook post was all it took for him to take his life then maybe his elevator didn’t go all the way to the top.

Burglar Discovers Dead Body In House According to The New Zealand Herald A burglar who discovered a dead body hanging in a Hamilton house is unlikely to face charges for the break-in. The 26-year-old man made the grisly discovery early yesterday morning while attempting to burgle the vacant house in the suburb of Fairfield. Hamilton Police city tactical coordinator Senior Sergeant Freda Grace said the burglar, who called police himself to raise the alarm, had been arrested but not charged. Mrs Grace said the man was released after helping police with their inquiries. The incident unfolded in the early hours of yesterday morning when the burglar stumbled across the dead body hanging in the dark. His screams alerted neighbors who also phoned police. It’s understood they thought the screaming was a domestic dispute. Mrs Grace said the victim had died hours before the burglary but if not for the break-in he may not have been found for days. She said the death of the man, whose age is unknown, was not being treated as suspicious and had been referred to the coroner. The burglar was known to police and Mrs Grace hoped the “weird” circumstances would alter his behavior. “Hopefully there will be a positive out of it and that he will decide it’s not the thing to do. I would be taking that as pretty bad karma.” Mrs Grace said the whole situation was incredibly sad”. “It’s sad for the guy who felt so bad that that’s what happened to him. Really the whole set of circumstances are just horrid.” She did not know if the dead man’s next of kin had been notified. Via NYDailyNews A man identified by employees as a former maintenance worker opened fire Tuesday inside a Detroit medical facility, sending workers and visitors screaming and rushing for the doors just moments before the building erupted in flames. Crews digging through the gutted Park Medical Centers building hours after the fire recovered the remains of a man and a woman, Detroit police said Tuesday night in a release. Authorities did not release the identities of the dead, pending autopsies, but police had been searching for 35-year-old medical assistant Sharita Williams and the fired maintenance worker, who relatives said was her ex-boyfriend. Williams’ mother, Antha Williams-Hill, told The Associated Press that one of her daughter’s co-workers told her that the man threatened her daughter inside. “He said, ‘You think I’m playing with you?’” Williams-Hill said. “He told the other girl, ‘I think you better get out of here.’ The girl left and said she then heard two shots.” Last week, Sharita Williams was granted a personal protection order against the man, according to Wayne County Court records. Williams-Hill said two days earlier, someone had emailed her sexually explicit photos of her daughter that also were posted on Facebook. The text accompanying the photos read: “How do you like Sharita’s new boyfriend?” her mother said. Dr. Stuart Kirschenbaum, a podiatrist who operated his private practice from the building for about 30 years, said he heard a security guard yell that the gunman “had taken Sharita and is shooting at other people in the building.” Destroyed in the blaze, Kirschenbaum said, was his collection of boxing gloves, personal letters and photos of boxing great Joe Louis. He estimated the memorabilia were worth about $100,000. Dwane Blackmon, Detroit police homicide inspector, declined to identify the maintenance man as the suspect but said everyone else inside except the woman and male suspect appeared to have escaped the blaze. Investigators were unable to quickly go deep into the one-story building, described by tenants as also having a basement, due to fears of the structure’s integrity. Williams-Hill said she was asked to go to the coroner’s office to identify whether the deceased woman was her daughter “because of the condition of the body.” She said her daughter had been dating the married maintenance man for more than a year, but their relationship was rocky and Sharita eventually began seeing someone else and even moved out of the city and into a Detroit suburb to get away. However, he refused to leave her alone, Williams-Hill said. Police and arson investigators on Tuesday interviewed people who worked in the building and relatives of Sharita Williams and the maintenance man. The body of Rodney King was discovered at the bottom of a pool by his fiancee last month, with the cause of death listed as “accidental drowning.” That may technically still be the case, but the official death report has come out and it’s undeniable now that the man whose 1991 beating sparked riots in Los Angeles stumbled to his demise at the result of numerous substances in his body. It’s unclear if he jumped or fell into the water, according to the coroner, but King was “in a state of drug and alcohol induced delirium” at the time of his passing. “The effects of the drugs and alcohol, combined with [King’s preexisting heart condition] probably precipitated a cardiac arrhythmia and [King], thus incapacitated, was unable to save himself and drowned,” reads the report, which adds that King was lying naked in the deep end of the pool by the time EMTs arrived. Cynthia Kelly attempted to rescue her fiance with items such as a pitchfork, a hoe and a vacuum pool sweeper – all of which surrounded the area, the report confirms – because she considered herself a poor swimmer and was afraid of jumping in.
Tony Scott , the director who helped define high-octane ’80s action with his movies Top Gun (1986) and Beverly Hills Cop II (1987), has died of an apparent suicide at age 68. The Wrap reports that Scott jumped from a suspension bridge in Los Angeles yesterday afternoon, and the coroner’s office has confirmed that the British-born filmmaker’s body was retrieved from the LA harbor. A suicide note was discovered in Scott’s car, and The New York Daily News is reporting that a battle with inoperable brain cancer may have led to his suicide. Scott worked for years as a commercial filmmaker before making his feature debut with The Hunger (1983), a Mr. Skin Hall-of-Famer that features Susan Sarandon and Catherine Deneuve in one of the hottest lesbian scenes in SKINema history. That sort of Sapphic heat naturally caught Hollywood’s attention, and box-office supremacy soon followed with films like Days of Thunder (1990), The Last Boy Scout (1991), True Romance (1993), Crimson Tide (1995), and Man on Fire (2004). Scott’s last film was 2010’s Unstoppable , a runaway-train movie starring Denzel Washington and Rosario Dawson . He is survived by his wife and two sons. Tragic news: British filmmaker Tony Scott , who directed films including Top Gun , Crimson Tide , Man On Fire , The Taking of Pelham 1 2 3 and 2010’s Unstoppable , died today after jumping from the Vincent Thomas Bridge between San Pedro and Terminal Island, multiple sources confirm. “Scott, 68, climbed a fence on the south side of the bridge’s apex and leapt off ‘without hesitation’ around 12:30 p.m., according to the Coroner’s Department and port police… A suicide note was found inside Scott’s black Toyota Prius, which was parked on one of the eastbound lanes of the bridge,” reported The Daily Breeze . Earlier in the day reports of a man jumping from the Vincent Thomas Bridge hit the web, although Scott’s identity was not confirmed until later Sunday evening. Scott, who had recently produced brother Ridley Scott ‘s Prometheus , had a number of upcoming films in production including Chan-wook Park’s Stoker , Scott Cooper’s Out of the Furnace , and Ridley’s Prometheus 2 . The Scott brothers also produced the TV series Coma , which is set to debut September 4.
