Source: Nomad / Getty 68-year old grandfather, William Hannah , has died after Salford Royal Hospital in the U.K. mistakingly filled his lungs with cleaning detergent instead of saline. The Hannah family’s lawyer told PEOPLE , During the procedure the doctor found there was no saline solution in the equipment trolley and so asked a healthcare assistant to pass this to him. However, the assistant became confused and instead of providing the doctor with the requested saline, accidentally handed him an unlabelled bottle containing detergent that they had prepared for cleaning equipment. The doctor then unknowingly used the cleaning solution to wash-out William’s lung. Following an internal investigation into the wrongful death, the Salford Royal NHS Foundation Trust concluded that the hospital did not deliver the “high standard of care” and offered its apologies to the family. Hearings for the case begin in February 2019. SMH, so sad.

We now know what killed Rapper Mac Miller. The The L.A. County Coroner’s Office ruled is death an “accidental overdose.” The toxicology findings found that the rapper died from “mixed drug toxicity,” specifically fentanyl, coke, and alcohol. The amounts of each substance wasn’t enough to be deadly but the combination of all the substances was lethal. Miller was found by his assistant and was pronounced dead on the scene. There was a list of drugs found at his house like Xanax, oxycodone, hydrocodone, and generic Adderall. She put out “Thank You, Next” about all the lessons all her exes taught her. She mentioned her relationship with Detroit rapper Big Sean as well as her failed engagement to comedian Pete Davidson. Her lyrics says “ Thought I’d end up with Sean / But he wasn’t a match / Wrote some songs about Ricky / Now I listen and laugh / Even almost got married / And for Pete, I’m so thankful / Wish I could say ‘thank you’ to Malcolm / ‘Cause he was an angel.” If you want to know about how Rihanna ain’t playing with 45… Let’s just say Trump should be checking his mailbox for a possible cease and desist. It’s been nearly two months since Mac Miller passed away of an apparent overdose at his home in San Fernando Valley. Now, the reports that Miller’s death was a direct result of his substance abuse have been confirmed by an autopsy and toxicology report. According to findings released by the L.A. County Coroner’s Office moments ago, Miller’s death was the result of “multiple drug toxicity.” Fentanyl, cocaine, and alcohol were all present in the 26-year-old’s system at the time of his death. According to TMZ, the amount of any one of those drugs would not have been enough to kill Miller, but the combination proved lethal. The report also contains new information about the discovery of Mac’s body, which was found by his assistant, who immediately called 911. According to the recording of that call and the statement the assistant gave to police, Miller was found on his bed in a “praying position.” In his 911 call, the unidentified assistant described Miller as “blue” and stated that blood was coming out of his nose. While a medical examiner previously described Miller’s death as a “mystery,” all signs pointed to a lethal overdose from the start. Police found a large number of pills in Miller’s home, including Xanax, Oxycodone, Hydrocodone, and generic Adderall. A rolled up $20 bill with a white powdery residue was discovered in Mac’s pocket. And sadly, the cause of death does not come as a surprise to those who knew the rapper best. Mac’s struggles with addiction were well-documented over the years. In May, Miller was arrested on hit-and-run charges after he crashed his car into a streetlight pole in Los Angeles and then fled the scene on foot. When police arrested Miller at his home, the rapper blew twice the legal limit in a breathalyzer test. The incident occurred shortly after Miller’s breakup with Ariana Grande. In response to the news, Grande tweeted, “Pls take care of yourself.” The medical examiner who performed Miller’s autopsy also documented his many tattoos in the above drawing obtained by TMZ. In a heartbreaking acknowledgment of his struggles with mental illness and addiction, the rapper sported tattoos that read, “So confused” and “Only so much time left in this crazy world.” Our thoughts go out to Miller and his friends and family during this difficult time. Getty Images Rob And Tyga Both Contribute To Blac Chyna’s Assistant’s GoFundMe Looks like Blac Chyna is still cordial with both her baby daddies, Tyga and Rob Kardashian. Chyna’s assistant, Lorena “Patty” Hernandez, recently suffered a major brain hemorrhage, and it looks like both Rob and Tyga have made sizeable donations to a GoFundMe page being used to cover her medical expenses and keep up with her children. Tyga donated $5,000 under his legal name, Michael Stevenson, while Kardashian donated $10,000. Though Chyna has yet to make a public donation, she’s majorly helping her assistant and friend Patty in other ways. She has “been keeping [Hernandez’s] children at her house just to help out, this has actually been going on since before Christmas. It’s really heartbreaking and everybody is trying to help out as much as possible right now especially Chyna,” said a rep for Chyna to Page Six . The GoFundMe page explains that Hernandez initially suffered “a stroke that left her partially blind” in July 2017 and was “diagnosed … with a brain AVM arteriovenous malformation that was present since birth.” On December 18, 2017, Hernandez then suffered “a severe head bleed that left her unconscious,” the page says. “After a cat scan and a couple of days, Patty was finally stable and [doctors] advised to terminate her pregnancy lowering the chances of another head bleed because another head bleed would leave her [permanently] disabled or dead.” Patty’s pregnancy was unfortunately terminated on 10 days after the incident. On January 11, 2018, Hernandez suffered another seizure, which left her in a coma. Officers received a call for help came that from Chyna’s Lashed Salon in Encino, California. Prayers go out to Hernandez and her family during this difficult time, it’s nice to see that Chyna has a support system that will reach out and help a friend in need.
